Referring Patients to the Hillandale Lab

Our Atlanta physician referral procedure has been streamlined to ensure that your patients are referred to our Lab as efficiently as possible. We have been a Quest drawing site since 1995, and we are pleased to serve you in this process.

When your patient presents at our outpatient registration desk, located on the first floor of the hospital at 5900 Hillandale Drive, he/she will submit a Quest lab requisition form to the registrar. The registrar will determine if DeKalb Medical Hillandale accepts the patient’s insurance.

If so, the patient will be registered so that his/her labs can be performed at our facility. If not, the patient will be drawn and the labs will be directed to Quest for processing.

To ensure that your physician referral request is expedited as efficiently as possible, please include the following information on your request form:

  • ICD-9 code(s)
  • A number where we can reach you for critical results or additional questions we may have regarding the lab test(s) you requested
  • Whether you prefer we send results directly to you or fax them to your office after hours.

You can also access your lab results via the DeKalb Medical physician portal. If you do not have access to the portal, please call the IS Help Desk at 404.501.5757. A Help Desk representative will obtain your information and email you the required forms to be completed and faxed back.
