Referring Acute Care Patients
at DeKalb Medical
There are three convenient ways to begin the process of referring acute care patients to DeKalb Medical 's Long Term Acute Care (LTAC).

- Online via Allscripts Referral system (participating healthcare facilities only)
An LTAC medical professional evaluates each referred patient to ensure that he/she meets admission criteria. We consult with the patient’s attending physician to determine medical appropriateness and will recommend alternative placement if criteria are not satisfied. Acute care services pre-admission screenings are free. To schedule a screening, call 404.501.6262.
