Appointments

Dr. David Holladay & Associates’ office is open Monday through Thursday, 9 a.m. to 5 p.m. and Friday from 9 a.m. to 3 p.m. We are accepting new patients. Please call 404.501.7700 to make appointments.

For your convenience, you can download our patient registration packet. Please complete the registration forms and return to our office at least 72 hours prior to your appointment if possible.

Cancellations and Late Arrivals

To offer the best service for all patients we ask that you give us 24 hours notice for cancellation of appointments. This makes it possible for other patients to be seen at that time.

If you are more than 30 minutes late to your appointment, we will reschedule your appointment.

If you need a prescription refilled, please call 404.501.7700 during office hours. You may also fax your request to 404.501.7737.
