DeKalb Medical Patient Rights and Responsibilities

As a DeKalb Medical patient, you have the right to:
- Healthcare services equally accessible to all persons without bias related to race, creed, sex, national origin, age or payment sources.
- Respectful, considerate and courteous care while preserving patient dignity.
- High-quality, medically indicated and appropriate care.
- A clear understanding of his/her health status, diagnosis and progress and of all recommended tests and procedures.
- Know the risks and benefits of any procedure recommended and any alternative treatments that are available.
- Participate in decisions about your care, refuse treatments or procedures within the limits allowed by state law and a clear explanation of the consequences of refusal or if you refuse treatment. to be treated.
- A discussion about the ethical issues of your case.
- Choose whether you receive any experimental treatments or procedures. You must be able to understand and consent to your participation in experimental therapies or research.
- Know if your treatments are effective or if there are any unanticipated outcomes to your treatment. your treatment.
- A safe and secure environment.
- Privacy within the health care setting, including your right to private conversations and private examinations by your physician. You also have the right to confidentiality about your medical care. We will not release your medical information without your permission.
- Complete an advance directive (the Georgia Living Will or the Durable Power of Attorney for Healthcare).
- An appropriate assessment and management of your pain.
- Talk with a hospital representative about your care or the way you have been treated in the hospital.
- Have your cultural, spiritual, personal value beliefs and social needs respected by your health care team. We will make very effort to accommodate your special needs, within the limits of hospital regulations regarding patient safety issues.
- A statement of all charges for health care services provided to you, as
well as available payment options.
- Know the identity of each member of your health care team, including your doctors.
- See your medical records.
- Know about the existence of any business relationship among your health care providers and to know that these relationships will not affect the quality of care you receive.
- Reasonable continuity of your medical care when being discharged home or transferred. We will help you coordinate the transition of your care.
- Access, request amendment to, and receive an accounting of disclosures regarding his/her own health information as permitted under applicable law.
- Have a personal family member or representative of your choice and/or your personal physician notified promptly on admission to hospital.
- Be free from all forms of abuse or harassment.
- Be free from seclusion and restraints of any form that are not medically necessary or are used as a means of coercion, discipline, convenience or retaliation by staff.
- Informed consent.
- Access protective and advocacy services.

As a DeKalb Medical patient, you have the responsibility to:
- Give complete, correct information about your medical complaints, past illnesses and hospitalizations medications and treatments, lifestyle habits and other matters that relate to your health.
- Inform us if you are in pain so that we can appropriately treat it.
- Tell your doctor or nurse if you think your safety is at risk.
- Be aware of and follow the hospital rules that have been carefully established to ensure your safety and comfort.
- Provide accurate information about insurance coverage and your ability to pay any expenses not covered by a third party. You are ultimately responsible for all fees for services provided to you.
- Provide a written copy of your advance directive (the Georgia Living Will or the Durable Power of Attorney) upon admission if you have one.
- Tell your doctor and health care providers about any changes in your condition.
- Tell your health care provider if you do not understand your treatment or any instructions given to you, or if you anticipate a problem in following your treatment plan once you are released from care.
- Follow the plan of care upon which you and your physician agreed and keep your scheduled appointments after you are released from care.
- Accept the consequences if you refuse treatment or if you do not follow your doctor’s medical instructions. These consequences may include your being directed to find another doctor or health care facility.
- Treat the property of others and of the health care system respectfully.
- Respect the rights of other patients and health care personnel. Show your consideration for others by controlling noise; limiting the number of visitors you have at any one time; smoking only in designated areas (which does not include public entrances to our facilities); and refraining from other behaviors that negatively impact others.
