| Your physician will write a discharge order in your chart and discuss your release with you. This may be a transfer home, with or without home health services; to a long-term acute care facility; a sub-acute facility; rehabilitation services; or a nursing home.
Discharge Planning
Our Medical Management Department staff has nurse case managers and social workers that can assist both you and your physician with the appropriate referral to all of the services mentioned above, as well as other community resources. Please call 404.501.1120 for more information.
Checking Out
Checkout time is 11 a.m. for patients with discharge orders. Please make arrangements to have your family or friend ready to pick you up on the day of discharge by 11 a.m. Nursing staff and Medical Management staff can assist you in making arrangements for other transportation, such as ambulance or taxi, should this be needed. If you are unable to leave the hospital by 11 a.m., you maybe be moved to a discharge area to wait until your transportation is available.
Please observe the listed checkout times if you have been discharged by your physician. If you haven’t left your room by the end of the checkout period, you may be charged for an additional day. This additional charge will not be covered by your insurance; however, if you are discharged by your physician after normal checkout times, but before midnight, you may not be subject to additional room charges.
